I would really like to use the questionnaire script on your site but i cant
find the download link to the php file that actions the script. Can you
please send me the file via email or help me to locate the questans.php
file on the site so that i can view the source.

This script does not actually need to be downloaded. You can submit your
questionnaires directly to it. However, you are welcome to have a copy. See
attached.
