I got quite a few of these requests, so I got to putting examples of the normal credit lines on my terms of use page:

The usual for corporate intranet is:
[Put name of script here] script provided for free by
>a href="http://www.howtocreate.co.uk">http://www.howtocreate.co.uk>/a>.
For normal corporate internet sites, the following should also be added:
Customers are not charged for use of this script.
